Blade Versus Bladeless: Pros And Cons Of Different Lasik Procedures

What exactly is the difference between blade and Bladeless Lasik, and what are the advantages and disadvantages of each type?
The main instrument used by an eye surgeon in Traditional Lasik surgery is called a microkeratome, which can cut a thin, ‘hinged’ flap into the cornea (the clear surface on the eye). This flap is then lifted to let the doctor apply the laser and reshape the eye for vision correction. By replacing this flap, the eye is allowed to heal very quickly.
But, another way of creating a Lasik Flap uses a type of laser (femtosecond) instead of a blade. Intralase is usually advertised as ‘bladeless’ or ‘all laser’ Lasik, though eye surgeons who favor the microkeratome (‘blade’) approach are correct when they say both procedures technically penetrate the eye’s surface.
Eye surgeons differ about the advantages and disadvantages of blade versus bladeless Lasik. What they do usually agree upon is the need for truth in advertising. Some medical facilities argue that where the word ‘bladeless’ is used, it somehow implies that tradition Microkeratome Lasik is more dangerous to the patient when in fact it is not considered more risky.
Back to the pros and cons for using either blade flaps or laser flaps. Some doctors prefer using the blade flap, because microkeratomes make the procedure go much faster and the suction needed to hold it up is about three seconds, whereas the suction using the laser method is at least 15 seconds; some medical professionals think the less suction used, the more comfortable it is for the patient.
Some medical professionals believe that it’s more likely a doctor could induce a ‘buttonhole’ flap (an incomplete flap with a hole in the middle), or even detached flap, with the microkeratome surgery, and that the laser technique is less likely to do this because one gets the same thickness of flap with a laser no matter what the curve of the cornea happens to be. The argument for laser continues with the assertion that the more curved the cornea is, the thinner the flap could be centrally with a blade slip and therefore the risk of a buttonhole flap is greater.
Also, a physician ostensibly has greater control in bladeless surgery, because the computerized portion of it makes it easier to visualize the flap, and to stop the procedure if necessary.
However, both detached flaps and buttonhole flaps are extremely rare in statistics compiled from expert surgeons. Experienced surgeons also concur that irregular flaps could occur with both techniques if the surgeon was rough with either the laser or microkeratome. Both tools are purportedly only as good as the surgeons who wield them.
Studies have shown that one disadvantage to the laser flap can be a slightly increased risk of more edema (swelling) of the flaps because of all the energy the laser requires to make the flap. This can possibly delay clearness of vision from a few days to a week, and does not occur with a flap made with ‘the blade.’ There are also some studies that note a low risk of light sensitivity resulting from use of the laser.
Some doctors recommend using blade versus bladeless Lasik With Glaucoma patients, because the time and degree of suction is less.
In both blade and bladeless Lasik, there can be complicated cases where the patients, because of unsuccessful previous refractive surgeries, have to have them fixed with the same technology. For example, water bubbles can form during laser flap creation can filter through a previous blade flap incision, blocking the focusing lens of the laser. So, a doctor will often recommend that once a type of Lasik is decided upon, it’s best to have the same type again if needed in the future.

I had wanted to get Laser Eye Surgery for the past year but was always worried about the risks (you know the Kathy Griffin story) but I finally got it done and have to say I am very happy with the results. No more glasses/contacts for me and the recovery period was a hassle but definitely worth it!!!

I doubt they’ll mind. They don’t even have to know. It won’t show up on a medical exam. They’ll just see that you have good vision without any corrective lenses, which should be good enough for them.

I had laser surgery but not for short sightened-ness for abnormal blood clots behind my eyes. I would find the best specialist you can just because it is your eye sight which as you’re probably aware is extremely important. I personally wouldn’t get it done as laser really hurt my eyes when I had it done but everyone is individual, just do some reasearch

hello-i had it done in 2002 on one eye I only needed to have done…It was great-u don’t feel a thing..it was the best thing i ever did for myself with my tax return that year. 1st off what part of country u live in as i lived in PHX then adn they have these places everywhere. 2nd some of them allow u to watch the procedure being done to someone else b 4 u do it…I personally had them close the viewing thing as I was also nervous at the time & didn’t want to b distracted by people watchin…But when u go in u have a preliminary screening. they measure and check your eyes etc..no biggee-i hate the glaucoma test where they pu a puff of air into your eye-i think that is worse..so after that u go back in few days to get r done…they gave us a valium like pill to keep u calm. which was great they put numbing drops in your eyes which is fine..they head us wait in room with tv & movies & took one by one to have the procedure done..each person was back like in 5 to 10 min-mayb 15 at longest. but when they take u they set u in the chair -they blocked off my other eye and put this thing in to spread my eye open..I didn’t feel a thing…then they make a slice & pull up that then the laser does its part…u do not feel it at all–u sorta smell a burnt scent & that’s it. they put the flap back over. U get up go to next room they give ya some dark glasses to wear…I did mine in the evening about 7pm when I left I had to walk across big parking lot & catch bus home…I could already read the billboards & street signs…I couldn’t believe the stuff I could see it was unreal…I went home of course & took the meds to sleep & went to bed…the next morning when i woke up I could see the pics on my walls and everything..I am still so shocked & impressed to not have to wear glasses or those damn contacts anymore that I can’t tell ya…I started wearing glasses in 6 th grade & am almost 50 now..I am 48 & i can see..it is so amazing to me still & i thank god for the technology & the proper use of it…so by all means each person has different experience to b sure-but there is no pain as long as u listen to them & do as they say…U will b so glad to not have them steam up (your glasses) in the cold, have rain aggravate u to keep cleaning off so u can see, no more fingermarks or smudges etc…plus the expense of glasses or contacts…It takes a while to get used to not having the weight of them on or the air hit your eyes but so worth it…all the cool sunglasses u can wear & see…..so do it….best advice I have…no lil kids when u hold them to play with or smudge them…so many benefits to just a small amount of u sitting in chair and less than 1/2 hour of your time-no pain—duh–makes sense to me…I really hope u make sure of clinic as well & if they object to u possibly viewing a procedure than find another one that will and talk to other patients while there who have had it done….take care & best of luck Miss Rocky
