I paid for both. It is expensive but it is worth it. My mother was legally blind tho not diagnosed as such for obvious reasons. She wouldn't have been able to get a driver's license etc... She just cept going to the eye doc and getting stonger glasses. After surgery her eye sight is great 20/30 and she does not need glasses for anything. Her surgery was either 6 or 7 years ago. My surgery was also wonderful. Mine was about 5 or 6 years ago. I think that i may need to go and get my eyes tested again. The surgery will only take care of what is wrong with your eyes at the time. It will not prevent your eyes from continuing to get worse if they are going to. Most docs recommend that you wait till your eye prescription hasn't changed much in last couple of checks. There for if you are wearing coke bottle lenses the improvement would be great.
